**Schedule:** Monday-Friday | 8:00 AM - 5:00 PM

**Work Setting** : Work From Home + 75% Local Travel

**Pay Rate:** $34-$36.37/hr

Adecco Healthcare & Life Sciences is seeking a highly organized, tech-savvy, and self-motivated **Healthcare Consultant Field Case Manager** to support Medicaid Long Term Care/Comprehensive Program members in the Tampa, FL region. This role blends remote work with regular in-field member visits, offering a meaningful opportunity to support individuals in their homes, assisted living facilities, and nursing homes.

_Training will be completed remotely via Microsoft Teams._

**Key Responsibilities**

- Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.

- Conduct comprehensive evaluations using care management tools and data review.

- Assess, plan, implement, and coordinate all case management activities to meet members' medical and social needs.

- Manage a diverse caseload, including medically complex and supportive members.

- Provide care coordination, including:

? Prior authorizations

? Coordination with PCPs and skilled providers

? Condition management support

? Medication review

? Community resource referrals

- Conduct multidisciplinary reviews to ensure optimal outcomes.

- Identify and escalate quality-of-care concerns through established channels.

- Use motivational interviewing techniques to promote member engagement and behavior change.

- Empower members to participate actively in their health decisions.

- Maintain accurate documentation while complying with regulatory and accreditation guidelines.

**Required Qualifications**

- Fluent in Spanish & English (reading, writing, and speaking) - Required

- Must reside in Tampa, FL, preferably East Tampa (zip codes: 33610, 33617, 33604, 33612)

- 1+ year of Case Management experience - Required

- Bachelor's degree required (Social Work degree or related field preferred; no nursing license required)

- Proficiency in Microsoft Office, including Excel

- Valid high school diploma or GED (verified)

- Strong verbal and written communication skills

- Ability to multitask and thrive in a fast-paced environment

- Highly organized, detail-oriented, and able to work autonomously

- Comfortable completing 75% of work in the field (homes, ALFs, SNFs)
