Roles & Responsibilities

  • Provide compassionate, patient-centered home health care to clients in the Muskegon area
  • Document care accurately in EPIC and complete required Trinity skills and specialty competency checklists
  • Perform personal care, basic clinical tasks and support daily living activities per care plan
  • Communicate proactively with clinical team members, supervisors and families to coordinate care
  • Follow infection control, immunization and safety protocols to protect patients and staff
  • Participate in required fit testing, respirator evaluation and routine health screenings

Qualifications

  • Current BLS certification is required
  • Experience or comfort using EPIC for documentation and care coordination
  • License/certificate verification as applicable for the role (per Trinity requirements)
  • Completion of Trinity Skills Checklist and specialty competency testing prior to start
  • Up-to-date immunizations and health screenings as listed (COVID-19, MMR, Hep B, Tdap, Varicella, Influenza, TB screen or IGRA, 10-panel drug screen)
  • Reliable transportation and ability to travel locally to patient homes

Education

  • High school diploma or equivalent required; CNA or other healthcare training preferred

Knowledge & Skills

  • Familiarity with EPIC clinical documentation and electronic charting workflows
  • Strong interpersonal and communication skills for patient and family interactions
  • Basic clinical knowledge of home health practices and safety procedures
  • Ability to follow care plans, infection control standards, and privacy regulations
  • Comfort with competency testing and ongoing skills validation
